Nowadays, human population is quite high and crowded in several city. Besides that, people who lived in developed nations, tend to have more pressure in work place. That makes them try to focus in their career development. Those factors accumulate, and brings human to new habits. People tend to delay to have a child, and focus to their career, especially for women. Eventhough most of the women want to bear a child, but pregnancy is a matter of problem when they are building their career. Thus contraceptive methods has become basic needs.

The contraceptive methods have been developed and recognized by our civilization since ancient time. There are several methods from behavioural till sterilization. Each methods has it own benefits and disadvantages.
In this article, i'm gonna focus on birth control pills.

Birth Control Pills
Birth control pills (also called oral contraceptives or “the pill”) are used by millions of women to prevent pregnancy. For most women, the pill is a safe and highly effective form of birth control.

How does the pill work?

Birth control pills contain hormones that prevent ovulation (the release of an egg from an ovary). These hormones also cause other changes in the body that help prevent pregnancy. The mucus in the cervix thickens, which makes it hard for sperm to enter the uterus. The lining of the uterus thins, making it less likely that a fertilized egg can attach to it. Together, these events make it unlikely that pregnancy will occur.

With typical usage of contraceptive pills, about 8 in 100 women (8%) will become pregnant during the first year of using this method. When used perfectly, 1 in 100 women will become pregnant during the first year. To be effective at preventing pregnancy, the pill must be taken every day at the same time each day. The typical use pregnancy rate is higher than the perfect use pregnancy rate for birth control pills because a woman may forget to take the pill, or a pill may not be absorbed (because of vomiting, for instance).

Birth control pills do not protect against sexually transmitted diseases (STDs), including human immunodeficiency virus (HIV). A male or female condom should be used to provide STD protection if you are at risk of getting an STD.

Kinds of Birth Control Pills
There are two basic types of birth control pills: 1) combination pills, which contain the hormones estrogen and progestin, and 2) progestin-only pills.

Combination pills
The combination pill has health benefits in addition to preventing pregnancy. The pill helps to keep bleeding cycles regular, lighter, and shorter and reduces cramps. It can be used in the treatment of certain disorders that cause heavy bleeding and menstrual pain, such as fibroids and endometriosis, as well as conditions caused by imbalances in hormones. Some pills may help control acne. Combination pills may also decrease the risk of cancer of the uterus and ovary and improve bone density during perimenopause.

Some women are concerned about weight gain while taking birth control pills. Researchers have found no link between taking combination birth control pills and weight gain.

Combination birth control pills increase the risk of certain conditions. Some of these complications can be serious. They include the following: Blood clots in the legs, Heart attacks and stroke (smoking greatly increases the risk of these complications), Gallbladder disease, and Liver tumors.

Thus before choosing a contraception methods, your doctor will do history taking and several examinations to make sure that you don't have any contraindication for the contraception methods. For combination pills, history of vascular disease, diabetes, and certain cancers become serious precaution.

Progestin-only pill or the mini-pill, has become a solution for those women, wo want to use the pill, but not suit the combination one, because of the vascular problems such as blood clot, history of heart disease, hypertension, etc.

Secret Behind Water Birth

Introduction
Water Bath Delivery or Water Birth is a method of giving birth, which involves immersion of the mother body in warm water.

Water Birth is believed to have less pain for the woman during the process of labor, beside that this method also associated with safer and less traumatic birth experience for the baby.

However, critics argue that the procedure introduces unnecessary risks to the infant such as infection and water inhalation.

With the increasing practice and promotion of delivery in water, make us have to think it carefully, whether this method is safer compare to other method.

Benefits
Less Painful Labor
I'm not a woman, thus i can't be pregnant. But, i am a medical doctor, and i have seen how a woman during the labor. Well, based on my observation and the references that pain during the labor process is a natural process, with the moderate until severe intensity of the pain. Actually, woman could overcome those pain, especially when she is being motivated about the successful delivery and healthy baby.

Pain during labor has become special problem, besides the 3P (Passage, Passenger and Power). Pain is uncomfortable sensation, thus can cause a psychological stress beside the physiological stress. Thus, most of the women during labor are seeking the pain reliever.

Based on the reference, Water birth is an effective form of pain management during labor and delivery. Water birth is a form of hydrotherapy which, in studies, has been shown to be an effective form of pain management for a variety of conditions especially lower back pain (a common complaint of women in labor).

Other options for pain control during labor and delivery are epidural anesthesia and intravenous opioids. Epidural anesthesia may be associated with a higher incidence of instrumental delivery with forceps. However, epidural anesthesia does not increase the incidence of cesarean section and has been shown to be a safe alternative for labor analgesia.

Otherwise, full immersion in water promotes physiological responses in the mother that reduce pain including a redistribution of blood volume, which stimulates the release of oxytocin and vasopressin, which also increases oxytocin blood levels.

No evidence was found that this benefit was associated with poorer outcomes for babies or longer labours. It has also been found that in waterbirths the buoyancy of the mother and the baby allow for a gravitational pull. This pull not only opens up the mother's pelvis but also allows the baby to descend more easily.

Less Traumatic
Water birth is believed to aid stretching of the perineum thus become less traumatic. Support from the water slows crowning of the infant's head and offers perineal support, which decreases the risk of tearing and reduces the use of episiotomy, a surgical procedure which can cause a number of complications.

Moreover, perineal trauma is reported to be generally less severe, with more intact perineums for multips (multiparas, or mothers who have had multiple births), but in some literature about the same frequency of tears for primips (primipara, or mothers who are having their first birth) in or out of the water.

Infection
Some people who disagree with water birth, believe that this method could increase the risk of infection.
Based on the study in Canada, there is no difference was noted in the low rates of maternal and newborn signs of infection in women with ruptured membranes. Because of the rigorous protocols for cleaning birthing tubs between labors (especially in hospitals), there is little (if any) risk of transferring bacteria from infant to mother or mother to infant.

Slowed labor
Because of the documented relaxing effects of water, laboring in water is sometimes associated with a decrease in the intensity of contractions, and is thus thought to slow labor. While home birth experts argue that this must be evaluated on a case-by-case basis, some hospitals have adopted a "5 centimeter" rule, allowing women to enter the tub only once the cervix has already expanded to 5 centimeters.

Conclusion
Delivery process is unique, special and important milestone in women's life. Most of the women experience the uncomfortable pain, thus become a special problem in labor besides the 3P. Thus make women, seeking pain reliever.
Water birth has become an alternative method of delivery for women, thus have been shown less painful and less traumatic.
Water birth has not shown increase risk of infection and prolonged labor.

So, after all, water birth delivery is safe for both mother and baby.

Vaginal Birth After Cesarean Section

Introduction
Vaginal Birth after Cesarean Delivery means that practice of delivering a baby vaginally (naturally) after a previous baby has been delivered through caesarean section (surgically).

A caesarean section leaves a scar in the wall of the womb, that makes uterine wall weaker than the normal uterine wall, so if the woman goes in labor in a subsequent pregnancy there is a higher than normal risk of a ruptured uterus. Because of increasing risk of normal vaginal delivery, most of the delivery after cesarean section was done by Re-cesarean.

The old concept "once a cesarean, always a cesarean." This concept is obsolete and should be change into "once a cesarean, always a hospital delivery."

Things to Consider about VBAC
Based on the epidemiological studies, showed that the number cesarean section in nineties was four times higher than sixties. Common indication for cesarean section: Dystocia, Fetal Distress, Malpresentation, and Uterine Scar.

There are several reasons why a woman may consider vaginal delivery compare to cesarean section, such as: shorter recovery period, lower risk of infection, and less blood loss. Also for women planning to have more children, VBAC may help them avoid problems linked to multiple cesarean deliveries. These problems include hysterectomy, bowel or bladder injury, and certain problems with the placenta.

Based on the offering benefits from the vaginal delivery, recently VBAC is developed or promoted by the health providers for the pregnant women.

A woman who has had a previous cesarean delivery has the following choices when planning how to give birth again:
- She can have a scheduled cesarean delivery.
- She can try to have a VBAC. If a woman wants to try VBAC and is considered a good candidate.

From the women who undergo VBAC, about 60–80% succeed and are able to give birth vaginally. But if problems arise during VBAC, the baby may need to be born by emergency cesarean delivery. For example, if the baby is not tolerating labor, or if labor does not progress, an emergency cesarean delivery may be needed. There are more risks, such as a greater risk of infection, with having an emergency cesarean delivery after TOLAC than having a planned cesarean delivery. The least number of risks occur with a successful VBAC.

Vaginal Delivery after Cesarean
Old concept from Cragin (1916), once cesarean always a cesarean delivery, is obsolete, Previously the technique of cesarean section was done by vertical incision of the uterine wall, recently is done by lower transverse incision of uterine wall. The lower transverse incision carried the lower risk of uterine rupture compare to vertical incision.
Cesarean Section

Unfortunately, it's not possible to tell what kind of incision was made in the uterus by looking at the scar on the skin. Medical records from the previous delivery probably include this information. If medical records are not available, a woman should understand the risks associated with all three types of incisions. It is still possible to have a VBAC unless it is highly suspected that the incision is a high vertical incision.

Women with elective cesarean with previous history of cesarean delivery should be motivated to undergo sterilization, since there is an increased morbidity and mortality for the women with increased number of cesarean section.

In VBAC, the usage of induction or stimulation of uterine contraction is not recommended and maybe dangerous for the women.
Pregnancy and Delivery method

Based on the report, the incidence of uterine wall rupture is quite high especially in the developing countries. In developed countries the incidence about 0-2%, compare to developing countries about 4-7%.

Selection for Vaginal Delivery after Cesarean Section
Patient Selection: in selecting the patient, we should know about the previous operation, VBAC is preferred for patient with lower transverse incision.

Contraindication for VBAC:
- Prolonged Labor (Dystocia), Malpresentation, Fetal Distress, and other conditions such as Placenta Previa, Cephalic Pelvic Disproportion, etc.
- Twin
- History of Uterine wall rupture
- unkown procedure of cesarean section, or High Vertical incision of uterine wall

Actually, there is no different between VBAC and normal delivery, but the observation and monitoring of uterine contraction and fetal condition is more intensive,

After all, the choice of delivery method is patient right or autonomy, and a medical provider should give a sufficient information to the patient, thus they could decide it well. A proper discussion with the medical provider is needed to get mutual understanding and agreement about the delivery method.
